1) Will the Oatey 43500 offset closet flange fix a toilet drain that is out of position?
That is what the offset is for. The 43500 offset closet flange gives 1.42 inches of offset between the pipe connection and the bolt ring, so a rough-in stub that landed too close to or too far from the wall can still be brought under the toilet. On top of that, the metal ring rotates, so you can spin the closet bolts into the right alignment after the flange is solvent welded. Between the 1.42 inch offset and the rotating ring, most out-of-position stubs get corrected without cutting the floor open and re-piping.
2) When should I use an offset closet flange instead of a straight one?
Use a straight flange when the drain stub is where the toilet wants it. Reach for the offset, like the 43500 with its 1.42 inch offset, when the rough-in missed: the stub is too tight to the wall for the fixture rough-in, or slightly off the centerline of the closet. Offsetting the flange is far cheaper than opening the floor and moving the pipe. Fasteners are not part of the flange, so pick screws suited to what you are anchoring into, wood subfloor or concrete. On an offset flange the extra anchor points matter more than usual, because the load from the toilet is not centered over the pipe.
4) Does the Oatey 43500 come with a test cap or a knockout?
No test cap. The 43500 is the version without one, so it will not hold water for a DWV pressure or standpipe test on its own. If the rough-in still has to be tested, either test before setting the flange or order the offset flange version that includes a test cap. Once you are past testing and setting finished floor, the no-cap version is the straightforward choice.
