It has a 2-1/8 inch top bore and a 2-1/8 inch bottom bore spaced 3-5/8 inches apart center to center, so it wraps a door already prepped for a key-in-knob lock and a deadbolt above it. If your knob and deadbolt are spaced differently than 3-5/8 inches, the holes will not line up.
2) Can a wrap plate repair a door that was kicked in, or do I have to replace the door?
A wrap around plate like this one is the usual repair for a split or torn lock edge. The stainless steel wrap covers the damaged area on both faces and the edge at once, reinforcing the bore area and protecting against another forced entry attempt. It covers a 4-1/4 by 9 inch area on the face, so damage confined to the lock and deadbolt zone is handled without hanging a new door.
3) Does the Pro-Lok wrap plate work on metal doors or only wood?
Both. The stainless wrap is rated for wood or metal doors and is used in residential, commercial and industrial openings, so a hollow metal apartment entry or a wood unit door are both in scope as long as the door is 1-3/4 inches thick.
4) Does the wrap plate replace my lock, or does the existing hardware go back on?
The existing hardware goes back on. This is a reinforcement plate, not a lock. The knob or lever and the deadbolt reinstall through the 2-1/8 inch bores with the plate sandwiched between the lock trim and the door face, which is why the 2-3/4 inch backset and 3-5/8 inch hole spacing have to match your prep.
