Center-to-center is 8-1/2 inches. If you are replacing an existing guard or pull, transfer that dimension to the door before you drill; the R116EDPH will only pick up existing holes prepped at 8-1/2 inches on center.

It is a heavy guard that mounts over the push bar area of a panic device and takes the abuse the device would otherwise absorb. The usual case is a door that sees carts, dollies, luggage, or gurneys, or a service exit where people shoulder through: the guard eats those hits instead of bending the pushrail or knocking the device out of adjustment. It does not replace the exit device or change how it operates. On a light-traffic office exit it is not necessary; on a loading dock, kitchen exit, hospital corridor or school egress door it is what keeps you from rebuilding the panic hardware every couple of years.

Commercial. Rockwood builds this line for institutional buildings, offices, retail, education, detention, sports facilities, hotels and restaurants, and the R116EDPH is a commercial-duty stainless part. Where you would be standardizing a facility, the single 630 satin stainless finish and the fixed 8-1/2 inch center-to-center hole pattern make it repeatable across a run of openings.
