Either way works. The 10PBS6X can be hard wired back to the automatic door operator, or paired with BEA's line of wireless transmitters so you skip pulling wire from the plate to the header. The wireless side works on 900, 433 or 300 MHz, so the transmitter and receiver you order have to be on the same frequency.
2) Do I need a surface mount box or a flush box for the BEA 10PBS6X?
Both are supported. The 6 inch square plate mounts to either a surface mount box (for masonry, glass mullions, or anywhere you cannot cut in) or a flush mount box where you can recess into drywall.
