1) Does the Thomas & Betts ALL-4 conduit body come with a cover and gasket, or do I order those separately?
The ALL-4 body is sold on its own (unit of measure Each), so the cover and gasket are separate line items. Thomas & Betts lists the die cast cover as CV-4 and the stamped aluminum cover as SCV-4, with a composition gasket GKV-4 or a neoprene gasket GKN-4. 2) Can I use the ALL-4 conduit body on EMT, or only on rigid conduit?
It is a threaded body for rigid and IMC conduit. EMT is unthreaded, so it will not thread into this body; you would need an EMT-to-threaded adapter or a fitting made for EMT instead.
3) Is the Thomas & Betts ALL-4 suitable for outdoor or wet location runs?

The threaded body maintains a weather resistant conduit system when it is used together with a cover and a gasket, so the body alone is not the whole story: pair it with the CV-4 or SCV-4 cover and the GKV-4 composition or GKN-4 neoprene gasket. Neoprene is the usual pick where the cover gets opened and reseated.
4) How many conductors can I pull through an ALL-4 conduit body?
The wiring chamber volume is 32 cubic inches, which is what you use for box fill calculations when splices are made inside.
