Trim only. The 1341 set gives you the plates, handles, and cylinder. The multi point mortise lock box is not part of it, so if the door does not already have a working multi point lock installed, that has to be sourced separately. This is swinging patio door hardware, so the usual case is dressing a door that already has the lock body in it.
2) What does Door Configuration #3 mean on Emtek multi point trim, and how do I know it matches my door?

The 1341 is Config #3, which puts the keyed American cylinder ABOVE the handle at a 4-1/8 inch center-to-center spacing. If the cylinder falls below the handle, or the spacing is different, a different Emtek configuration applies.
3) Can I key the Emtek 1341 patio door trim to match my existing house key?
Yes, within the keyway families offered. The 1341 takes an American cylinder and ships in either the Schlage keyway (the standard choice) or the Weiser WR5 keyway, so pick the one your existing house keys use. Keying is single cylinder, meaning key outside and thumb operation inside. Keyed alike or keyed different is selected on the order, and there is a field to key to a specific code if your locksmith or builder gave you one.
4) Do I have to specify handing when ordering Emtek 1341 multi point trim?
Handing is required, right hand or left hand, and there is no reversing it after the fact on this trim. Stand outside the door, on the side you would use the key.
