The Oatey 90104 Johni-Bolts are 1/4 inch diameter by 2-1/4 inch long closet bolts, which is the standard size for securing a floor mounted toilet to a closet flange. They drop into the slots of a typical flange, and the captive washer holds each bolt upright so it doesn't fall through the slot while you set the bowl.
2) What comes in a pack of Oatey 90104 closet bolts, and is it enough for one toilet?
Each plastic bag holds a pair: two bolts with captive washers, two additional washers, and two brass acorn nuts. One pack sets one toilet, since a standard floor mounted water closet uses two bolts.
3) Are these toilet bolts solid brass or just brass plated steel?
Solid brass, bolts and acorn nuts both. That matters in a wet location under a toilet base, where plated steel bolts rust and seize so badly the next person has to cut them off.
4) Can I use these for a new toilet install, or only for replacing an old one?
Both. The 90104 Johni-Bolts are used on new installations and on replacement of existing water closets, securing the bowl down to the flange.

If the flange sits well below the finished floor and you are stacking extension rings or multiple wax rings, the threaded length left above the bowl base shrinks, and a longer closet bolt is the better call. Measure from the top of the flange slot to the top of the bowl's bolt hole before you commit.
