1) What does a Nibco 4816 cleanout adapter connect to on each end?
The 4816 is a spigot x cleanout adapter with a plug. The spigot end goes into the hub of a PVC DWV fitting, and the other end is the threaded cleanout opening that takes the supplied plug. It's solvent weld on the pipe side, so no threading or gasket is involved on that joint. Schedule 40 PVC in white.
2) What pipe sizes does the Nibco 4816 PVC cleanout adapter come in?

3) Does the Nibco 4816 cleanout come with the plug, or do I order that separately?
The plug is included. The 4816 is sold as an adapter with plug, so you get the threaded cleanout body and the plug that closes it. What you do need on hand is the solvent cement and primer for the spigot joint, which are not part of the fitting.

5) Do I need a spigot cleanout adapter or a hub one for my PVC drain line?
It comes down to what you are joining to. A spigot end is sized like pipe, so it drops into the hub of a fitting already in the line, a wye or a coupling, without adding a second fitting. If you are coming off cut pipe instead, you want a hub (socket) cleanout that slips over the pipe. The 4816 is the spigot version, cemented in place.
6) Can I thread the Nibco 4816 cleanout onto the pipe instead of gluing it?
No, not on the pipe side. The 4816's pipe connection is solvent weld only, so that joint gets primed and cemented. The only threads on the fitting are at the cleanout opening for the plug, which is what you back out later to rod the line.
7) Is a PVC cleanout going to corrode or scale in a waste line the way metal does?
No. PVC will not rust, scale, pit or corrode, and it holds up to chemical attack, which is why it is the usual choice for drain, waste and vent runs in multi-family and institutional buildings where drain cleaners get used regularly. The 4816 body and plug are PVC throughout.
