It repairs and reinforces wood or metal doors that have been split or pried at the lock, and it works with key-in-knob locksets or deadbolts.
2) How much of the door face does the Pro-Lok wrap plate cover?
The plate is a 4-1/2 x 4-1/2 inch wrap, so it covers that area on each face of the door around the lock and bridges the edge in between. That's enough to hide and reinforce typical pry damage at the latch, but if the split runs further up or down the stile you'll want a longer wrap.
3) Can this be used on a metal door, or is it only for wood?

No. The Pro-Lok wrap plate has a single 2-1/8 inch bore over a 4-1/2 x 4-1/2 inch field, so it protects one lock point. An opening with a knob and a deadbolt on separate preps needs either two wraps or a taller double-bore wrap that spans both holes.
