1) Will the Arrow MLX81 entrance lever fit a 1-3/8 inch door, or does it need a 1-3/4 inch door?

9) I need a lever that always stays locked outside and keys in - is the MLX81 right?
No. The MLX81 entrance function unlocks with an inside turn-button and stays unlocked until the button is manually released, so the outside lever is free whenever someone leaves it that way. For an outside lever that is always locked and always keyed, use a storeroom function - the Cal-Royal AG05 Grade 2 storeroom lever with deadlocking latchbolt or the Hager 2580 Grade 2 storeroom lock with Withnell lever.
