Trim only. The 1114 set is plates and handles; the multi point mortise lock box is a separate purchase. If you're replacing worn or damaged handles on a swinging patio door and the gearbox in the door still works, this is the right part. If the lock itself has failed, you need the mortise multi point lock body too.

Configuration #1 is the non-keyed setup: a fixed (dummy) handle on the exterior and an operating handle on the interior. It's used on the semi-active leaf of a pair of swinging patio doors, the one you don't normally walk through and don't key from outside. If the door is your main entry and needs a key cylinder from the exterior, configuration #1 is the wrong trim.

3) Will Emtek 1114 trim fit my Andersen or Marvin multi point patio door?

The 1114 trim is referenced to multi point systems from Atrium, G-U, KML by Andersen, Andersen, Marvin Integrity, Paddock Lockmaster and Superior. The number that decides it is the center-to-center spacing between the handle spindle and the cylinder location: this plate is 3-5/8 inches CTC. Measure that on your existing door before ordering, because a door from any of those makers built to a different spacing will not line up.

4) Do I have to specify handing when ordering Emtek 1114 multi point trim?

Handing is required, right hand or left hand, and it isn't field reversible on the order form. Stand outside the door: if the hinges are on your right it's right hand.
