1) Does the Emtek 1181 multi point trim include the multi point lock body, or just the handles and plates?
Trim only. The 1181 set gives you the plates, the handles, and the cylinder. The multi point mortise lock box itself is not included, so if you are replacing a whole system you still need the lock body that matches the door. This is the part to buy when the multipoint gearbox in the door is fine and the handles or plates are worn, bent, or the wrong finish.
2) Will Emtek 1181 trim fit my Andersen or Marvin patio door?

The deciding number is the hole spacing: the 1181 is Door Configuration #1 with 3-5/8 inch center-to-center between the handle hub and the cylinder. Measure from the center of the existing handle spindle hole to the center of the key cylinder hole on your door. If that comes out at 3-5/8 inches with the cylinder below the handle, this trim lines up.
3) What is the difference between cylinder above the handle and cylinder below the handle on Emtek multipoint trim?
It is not a style choice, it is the hole layout already drilled into your door. The 1181 is the below-handle version: the key cylinder sits under the lever at 3-5/8 inch center-to-center. If the key on your existing door sits above the lever, the 1181 plate will not line up with the prep and you want the above-handle configuration instead. Check the door before you pick, not the picture.
4) Do I have to specify handing when ordering the Emtek 1181 Concord trim?
Yes, handing is required, right hand or left hand. Stand outside the door, on the side the key goes in: if the hinges are on your right, it is right hand.
