1) Does the Emtek 1251 multi point trim include the multi point lock body, or just the handles and plates?
Trim only. The 1251 set includes the plates, handles, and cylinder, but the multi point mortise lock box is not part of it. The configuration number and the CTC are what actually decide fit, so measure hub to cylinder on your existing trim: if it isn't 3-5/8 inches with the cylinder above the handle, this is the wrong config.
3) What is the difference between cylinder above the handle and below the handle on Emtek multi point trim?
It's the physical layout of the plate, and the two are not interchangeable. The 1251 puts the keyed American cylinder ABOVE the handle, spaced 3-5/8 inches center to center. Your door's multi point gearbox has the cylinder tailpiece hole in a fixed spot relative to the handle spindle, so if yours sits below the handle, an above-handle plate won't line up with either hole pattern. Pull the existing trim and look before ordering.
4) Can I key the Emtek 1251 patio door trim to match my existing house keys?
