Trim only. The 1621 set gives you the two plates, the handles, and the cylinder. The multi point mortise lock box is not included, so you either keep the lock body already in the door or source one separately. That is the usual arrangement on a swinging patio door retrofit: the multi point gearbox stays in the slab, and you change the trim on both faces.
2) What is the center-to-center measurement on the Emtek 1621 arched trim, and will it line up with my existing multi point door prep?
Center-to-center is 3-5/8 inches, measured between the handle hub and the cylinder hole. The plates run 2 by 10 inches. It is built to Door Configuration #6 with the cylinder hub below the handle, so the existing prep in your slab has to be a 3-5/8 inch spread with the key hole under the lever, not above it. If your door is drilled to a different config or spread, this trim will not drop on.

4) Can I key this Emtek patio door trim to match the Schlage or Kwikset keys already on my house?
Yes, within two keyways. The 1621 takes an American cylinder and ships either with a Schlage keyway (the standard choice) or a Weiser WR5 keyway, which is the Kwikset-family keyway. Pick the one your existing keys use. Keying is set at order time: choose keyed different or keyed alike, and if you have a bitting code from your other locks you can enter it as keying to code so the patio door runs on the same key as the rest of the openings.
